MRN vs MRI

MRN = Magnetic Resonance Neurography

MRI = Magnetic Resonance Imaging

 

An MRN is a modification of MRI for the direct imaging of peripheral nerves. It differentiates between

nerves and muscles better than MRI. The difference in the signal intensity of

intact nerves relative to that of muscles is amplified and injured nerves are characterized by both

  1. Increased signal intensity and,

  2. Enlarged size of the nerve structures

The proximal pudendal nerve segment is easily seen on MRN; however it is not possible to confidently identify the distal branches of the pudendal nerve even after surgical marking.

  1. Proximal (close) Portion   -1st Branch - Rectal Nerve

  2. Distal (farthest) Portions  - 2nd Branch - Perineal Nerve

                                               - 3rd Brach - Dorsal Nerve

​

The pudendal nerve gives rise to the inferior rectal  nerve as soon as it enters the canal (also called the inferior rectal nerve).  When it reaches the distal portion of the canal, the nerve bifurcates and gives rise to two more branches: the perineal nerve and the dorsal nerve of the penis/clitoris.

Radiologist may comment on the lesions along the course of the pudendal nerve, but not to over-diagnose

pudendal neuropathies, especially in the distal segments, which cannot be confidently seen.
